During the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported that Paradise Creek Health and Rehab of Cascadia was operated by Cascadia Idaho Operations, LLC. Cascadia Idaho Operations, LLC has managed the Latah County nursing home since May 1, 2023.

The facility operates as a for-profit entity.

For the same period, the facility was given an overall CMS rating of 2, below Idaho’s statewide average rating of 3.2 for nursing homes. CMS ratings reflect assessments of health inspection results, staffing, and resident care quality.

Data showed that Paradise Creek Health and Rehab of Cascadia cared for about 45.8 residents on an average day in the first quarter of 2026, with a capacity of 63 beds. The facility is not affiliated with a hospital.

Financial records for Q1 2026 indicated $90,362 was paid in 13 fines, with 13 total penalties imposed.

There were two for-profit nursing homes in Latah County, Idaho, during this timeframe.

Estimates from the U.S. Census Bureau show the population of older adults in the United States is growing, while younger demographics decline. From 2020 to 2024, Americans age 65 and over grew by 13%, and those under age 18 decreased by 1.7%. The nation’s senior population has surged for a century, increasing from 4.9 million in 1920 to over 61 million in 2024.

A 2021 study estimated that nearly half of Americans in this age group—about 28 million people—are expected to need long-term services or assistance, with more than one-third projected to require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
